Hi All,

    • I haven’t found a FOAM post on this, but I’ve reviewed some restricted access sites such as Emergency Medicine Cases and here’s a quick summary:
    • Symptoms : The inhibition of re-uptake of norepinephrine, dopamine and serotonin leads to hypertension, tachycardia, hyperthermia, agitation, seizure and coma. The sodium channel blockade leads to QT prolongation and VT. The stimulation of endothelin and nitrous oxide leads to vasospasm, vasoconstriction, further hypertension, potential STEMI and aortic dissection.
    • Treatment : Benzodiazepam, Nitrites, Bicarb, active cooling, Phentolamine, CCB’s. Avoid Beta-blockers due to unopposed alpha adrenergic effects.
